This upload is only to update the logo on these forms. I will address other issues with the forms in separate pull requests.

re: membership application and withdrawal request the fonts are different for "Membership Application" and "Member withdrawal request". I don't care much for either font but I think we should choose a font the works for both the "source" odt and the final pdf? |

If all of the .odt and .pdf forms are updated and pushed to the cottage14-patch2 I can review them. |
