Description
This PR adds a link to a newly created general feedback form called 'Give feedback to NHS Notify' to our phase banner, replacing the 'News' notification that it currently provides about NHS App messaging.
Having the feedback form in the banner follows patterns used in both GOV.UK and NHS.UK, specifically the phase banner component.
It also means that users can access the feedback form on any page.
Context
Feedback from users about how NHS Notify could be improved is fragmented and given in different formats at different parts of the overall user journey. This is a problem because product and development teams do not have a consistent source of information that they can draw from to inform product development work
There is also a problem that non-standard feature requests come at multiple points in the user journey, and do not always make to product leads for prioritisation. This leads to teams delivering features that are non-standard without the necessary oversight and discovery into the value and need of the feature. This means that developer resource might not be being used as effectively as it could be.
