CollectionRelType form using default

Describe the bug
I set up a collection relationship using the XML from speciforum's documentation. The CollectionRelType form is using the default and is not functional.

• name is a pick list instead of a text field making it impossible to establish a new type without a workaround
• qcbx for left and right side fields show no results and the search icons to open query builder don't work so you can't assign a collection
• specific to 7.8.13, the qcbx search icons to open query builder still work so you can still assign a collection

Additional Context:
The workaround for creating relationship type is to use view/collectionreltype/1 instead of view/collectionreltype/new to force-populate the field (thank you @grantfitzsimmons)

This has progressed :-( left/rightCollectionSide qcbx's are no longer functioning at all.
Changing the URL ending to /1/ instead of /new/ doesn't help anymore either.
The schema config and XML revealed no insight into the broken fields.
Changing utitype=textareabrief for the name field in the viewdef was also not helpful.

• no query combo boxes whatsoever
• qcbx's that are available on the New Collection Relationship form search Cat # and thus can't be used to configure a collection relationship

The Collection Relationship form should have query combo boxes pointing to Collection Objects. That's what that table joins.
