Include the actual CSV file for example glossary entries in an appendix

This change was discussed in #205 with the benefits of:
- keeping our "pretty" visualization
- being able to make it even prettier by using an asterisk or something to indicate that D is implicit in the parent example
- providing potential users of these tools with the information they need to use them
