Error: EMFILE: too many open files

I'm running node using Visual Studio Community on Windows, and get a lot of

Error: EMFILE: too many open files

errors. More specifically, most trigger a

SourceMaps: could not preload for generated script

error. It's well known that you shouldn't try to open too many files simultaneously on Windows. Preferably, this would be refactored so that this is not parallelized to extreme (the default ceiling is 10k I believe?).

A suggestion is to use https://www.npmjs.com/package/graceful-fs instead of the built in fs package in utils: https://github.com/microsoft/vscode-chrome-debug-core/blob/master/src/utils.ts#L360
