docs(guide): improve formatting and spacing in documentation

- add blank lines for better readability in markdown sections
- fix inconsistent indentation and spacing in code examples
- ensure all sections have clear separation and structure
- update prompt construction guide with consistent formatting
- append missing newlines at end of files
