Create a template to capture information for new Spot sources

We need to be careful about what we try to report, as this could quickly get out of hand. To enable others to provide the right information, we should define what information we need in a template. This template should be added to the wiki/ manual.

What is the absolute minimum we need?
What else would we like to have?
