update file structures and synchronization workflows between the Codelist repository on Github and the codelist server. #14
Description
the URI defined for the lists and list items must match the actual URIs
- we want to correct all codelists to be at https://codelist.commonapproach.org/[codelistname]; this should make it easier to have one codelist per folder in the repo, and simplify the vhost rewrite rules we have in place now.
- old codelist URIs can redirect to the new versions
- once new lists are in place, old codelist files should be removed from the server and Github.
