Update app to use Slack's granular permissions

Use slack's V2 Oauth2 implementation that allows for granular permissions.

Note: The app code itself has been prepared to accept the new v2 auth flow, it just needs to be tested in dev and have all current "add to slack" links updated to the new v2 link.
