Update interest form for existing mentors #628
Description
We need to update the interest form we send to existing mentors for 2026 availability

We need to send emails to the mentors to fill the form for 2026:

Note: this does not guarantee all mentors that respond will be considered for 2026 (we'll be reviewing our current mentors directory)
