Lesson assessment results (sometimes) fail to save

Each lesson ends with an assessment (quiz), but sometimes it get stuck at "Quiz complete. Results are being recorded." without recording the results. Sometimes it works after reloading the page (and retaking the quiz) but not always. Recording the quiz is required to continue on with the course, so this is problematic.

I'm unsure if the following console error is the cause:

jquery.min.js?ver=3.7.1:2 POST https://learningdita.com/wp-admin/admin-ajax.php 500 (Internal Server Error)

In any case, this is referencing the various assessments at https://learningdita.com/quizzes/* and while I can't tell which conditions allow it to record (versus not), we'll see if more information reveals itself. Just now a quiz failed to record twice in a row which finally convinced me to write this report, but it did record on the third try (and did not report the above error).
