Add Patrick to maintainers and update GitHub profiles (#3533)

Since I couldn't reasonably reach out to everyone to ask if they wanted
their Twitter or website, I replaced our links with just GitHub
profiles, from which people often link their Twitter or website (and
it's up to them to do so). Also fixed Tyler's GitHub. This is more
useful too because now the handles match GitHub handles.
